For insurance liability purposes, application for membership excludes the following breeds of dogs: Akita, Chow Chow, Doberman Pinscher, Pit Bull, Rottweiler and Wolf Hybrid.

A copy of each dog's veterinarian records for which you are making application is required. Please submit a scan of the document via email to info@hillcrestflynn.com or fax to 724-346-1809 following the submittal of this application.
