A copy of  this signed and dated waiver must be submitted in person to Hillcrest Dog Park and Wellness Center upon use.
The waiver states:
I understand that membership of my dog and dogs is not without risk to myself, members of my family or guests as well as my dog and dogs when using Hillcrest Dog Park and Wellness Center or any function at Hillcrest-Flynn Pet Funeral Home and Crematory and Hillcrest Memorial Park, Inc.
I hereby waive and release Hillcrest Dog Park and Wellness Center, and hillcrest Memorial Park, Inc. its employees, owners and agents from any and all liability of any nature or kind, for injury or damage which my dog, myself, members of my family or guests may suffer, however caused, including specifically, but not without limitation, any injury or damage while using the dog park, the surrounding picnic and bench areas, the parking lot or while attending any training session or other function of Hillcrest Dog Park and Wellness Center as well as Hillcrest Memorial Park, Inc.
In consideration of and as inducement to the acceptance of my application for membership, I hereby agree to indemnify and hold harmless Hillcrest Dog Park and Wellness Center and Hillcrest Memorial Park, Inc., its employees, owners and agents from any and all claims, or claims by any member or guardians of my family or any other person on the grounds or the surrounding area thereto as a result of any action by any dog, including my own.
Addendum: For the protection of your dog or dogs and all of the dogs who are members of Hillcrest Dog Park and Wellness Center, we require that all dogs registered for membership be current on their licenses, vaccinations and rabies.
